I authorize HHC to set up selected utility accounts on my behalf and charge $50 one-time fee per utility.

I understand HHC will act as a liaison and is not responsible for delays or issues caused by the utility provider.

I understand that if incomplete or incorrect information is submitted, setup may be delayed

I understand this is a one-time setup of utilty accounts on behalf of the owner, including phone/email coordination, documentation uploads, and confirmation of service details. IF a field visit is needed for utility activation there will be an additional fee of $75
